New clients, please print and sign all  forms except “Release of Information” and fill them out before your first appointment. Doing so avoids having to spend our time together filling them out. You will have to sign in three places; twice on the guidelines/contract form and once on the HIPAA form. Also, after viewing these forms, you may want to call your insurance company to clarify your benefits.
